Discharge Planning & Coordination

Hospital and Nursing Home Social Workers and Discharge Planners have large case loads.  They often don't have the resources or time to dig deeply and find the best long range plan for the  patient.  This is where we come in.  Whether the client is in a hospital, nursing home, or acute rehabilitation facility, we work closely with the social workers and case managers to ensure a safe and smooth transition back home or to the right  independent or assisted living community.

It is important to know that we have personally screened and vetted all communities to which we refer clients. We verify credentials, health grades, types of care offered, and history of violations before we refer any client to a residential living facility.

We know what each community's specialization is and we base our recommendations on the best fit for each client’s care needs.
